The PA CHW Task Force Billing, Reimbursement and Sustainability Committee is conducting a survey of CHW employers to learn how they are financing their CHW positions. The survey should take no more than 5 minutes to complete. Only one person per employer should complete the survey. Survey responses will be analyzed and summarized by the Southwest PA CHW Collaborative, PA CHW Task Force, and PA CHW Association.
